Job summary

Baptist Health - Central Alabama is seeking a Discharge Planner (MSW) to coordinate care for an assigned caseload in a hospital setting. You will collaborate with physicians, nursing staff and other health professionals to ensure efficient utilization of resources and quality post-discharge plans.

Responsibilities include assessment, planning, care coordination, advocating for options and services, and documenting outcomes; you may take on additional duties within the role's scope.

Qualifications

• Masters Degree in Social Work required.
• Active LMSW or LICSW required or obtain within 12 months in position.
• CCM or ACM Case Management certification preferred.

Responsibilities

• Facilitate efficient utilization of clinical resources and quality post-discharge plans.
• Provide assessment, planning, care coordination, evaluation and advocacy for options and services.
