Qualifications

• Master's Degree in Social Work is required.
• Active LMSW or LICSW license required or to be obtained within 12 months in position.
• CCM or ACM Case Management certification preferred.

Responsibilities

• Coordinate care for an assigned caseload through collaboration with physicians, nursing staff, and other health professionals.
• Facilitate efficient utilization of clinical resources and financial outcomes.
• Provide assessment, planning, care coordination, evaluation, and advocacy for options and services.
