Job summary

Health Link Home Health and Hospice is seeking a compassionate Medical Social Worker (MSW) for San Francisco and surrounding areas. You will collaborate with a dedicated team to support patients and families in the home health setting.

The role offers modern tools, a supportive back-office, competitive pay, and benefits including health, 401(k), and PTO, with a flexible schedule and overtime eligibility. LCSW licensure, a valid CA driver’s license, and CPR are required.

Responsibilities

• Assist in the admission process by performing an initial evaluation, assessing psychosocial status, and identifying socioeconomic and emotional factors that impact the treatment plan.
• Collaborate in the development and implementation of the interdisciplinary patient care plan.
• Consult with the attending physician on any changes to the treatment plan.
• Maintain and submit required clinical documentation, including initial evaluations, care plans, and progress notes.
• Provide monthly supervision to the social work assistant.
