Job summary

SummitRidge Hospital in Lawrenceville, GA is seeking a weekend PRN Social Services Case Manager with recent behavioral health experience to provide clinical social work support across hospital programs.

You will deliver social services, assess patient needs, and collaborate with an interdisciplinary treatment team under the Social Services Department and unit leadership. A master's in social work or related field is preferred, with licensure encouraged.

Essential Job Functions include but are not limited to providing clinical social work expertise to hospital wide programs; deliver social services in assigned areas according to patient needs; adhere to standards of patient quality care and work as a member of the interdisciplinary treatment team within guidelines set by the Social Services Department and the individual unit Program Director.

Qualifications

Minimum Qualifications:

Master's degree in Social Work, Psychology or related field with specialization in mental health. Current professional licensure is strongly preferred. Recent experience in individual, group, and family therapy. Ability to assess the needs and provide the required range of treatment modalities and interventions.

Preferred: a minimum of one year experience in discharge planning, clinical case management or a similar position in a community based agency setting which provides experience in referrals for patients.
