Job summary

Endeavor Health seeks a Care Coordinator - MSW to support patients with chronic illness across a multi-site system. The role involves coordinating care, providing social work services, and partnering with care managers, physicians, and nursing teams to ensure smooth transitions and optimal outcomes.

Per diem schedule with hours Monday–Friday, some weekends and holidays as needed, and potential travel to Evanston, Glenview, Highland Park, Skokie or nearby Swedish facilities.

Qualifications

• Master’s Degree in Social Work required and IL licensure as LSW/LCSW.
• 1–2 years of social work experience in acute care preferred.

Responsibilities

• Coordinate with care managers and multidisciplinary teams to support chronic illness patients.
• Adhere to evidence-based care management standards and cost-effective interventions.
• Coordinate between departments, clinics, and community resources for safe transitions of care.
• Assist patients and families to develop self-management skills and improve quality of life.
• Provide support services to Endeavor patients and value-based members.
• Participate in safety, budget, and supply management within practice guidelines.
