Job summary

Hoag Memorial Hospital Presbyterian in Newport Beach, CA seeks a Care Manager I (Master's in Social Work) to coordinate patients’ needs through the continuum of care from pre-admission to post-discharge planning.

This role collaborates with physicians, nurses, and community agencies to identify and arrange appropriate care, review assessments, develop discharge plans, and support high-risk populations; position is Per Diem.

Qualifications

• MSW required; one year MSW experience in acute healthcare preferred.

Responsibilities

• Coordinate patients’ needs through the continuum of care from pre-admission through post-discharge plans.
• Collaborate with physicians, nurses, clinical staff, and community agencies to identify and arrange appropriate care.
• Review clinician assessments and patients’ financial, family and psychosocial support to develop comprehensive care and discharge plans.
• Review records for admission status, level of care, payer source, UR contracts to validate billing; provide psychodynamic intervention and crisis counseling as needed.
• Document patient issues, contacts and plans in medical records; serve as mandated reporter for elder, child, and spousal abuse; support high-risk, homeless, and mentally ill populations.
