Job summary

Mosaic is seeking a dedicated Mosaic at Home Coordinator/Case Manager in Denison, IA. The role involves office-based coordination with frequent home visits within the Denison area, travel up to two hours, and an agency vehicle or mileage reimbursement.

Qualifications

• Bachelor's degree in human services or related field plus minimum two years of related work experience.
• Previous experience with people with intellectual disabilities is a big plus.
• Valid driver's license in good standing (required for home visits).
• Strong communication, relationship-building, problem-solving and computer skills; HCBS regs knowledge is a plus.

Responsibilities

• Build epic connections with individuals, families, host home providers, and direct care workers.
• Be a dream champion by helping individuals articulate goals and needs and ensuring their voices are respected.
• Monitor relationships between individuals and home providers to ensure positive, supportive matches.
• Architect personalized plans with the service planning team, aligning wishes with policy requirements.
• Keep it real in the field by visiting homes and environments to oversee service delivery and contract compliance.
• Be the safety net by providing on-call support to providers and direct care workers.

• Be a Dream Champion:Help individuals express their goals, dreams, and needs. You're there to make sure their voices are not just heard, but loudly respected.


• Relationship Builder:You’ll monitor the relationships between individuals and their home providers, ensuring the pairing is a perfect, positive, and supportive match.


• Architect Personalized Plans:Collaborate with our service planning team to create Individual Support Plans. You’ll weave together a person's unique wishes with the necessary policy and regulatory requirements.


• Keep it Real in the Field:Oversee service delivery and contract compliance by hanging out where the magic happens. You’ll conduct regular home and service environment visits.


• Be the Safety Net:Provide on-call support to our wonderful providers and direct care workers when they need a helping hand or a quick problem solved.

What You’ll Bring to the Table:

• The Degrees & Deeds:A Bachelor’s degree in human services or related field ispreferred, along with a minimum of two years of related work experience.
