Mosaic in Council Bluffs is hiring a Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities in Pottawattamie County. This hybrid, salaried role includes on-call support and travel for home visits with mileage reimbursement.

You will build relationships, champion personal goals, and develop Individual Support Plans with our planning team to ensure compliant, person-centered services in community settings.

Qualifications

• Bachelor’s degree in human services or related field required.
• Minimum of two years of related work experience preferred.
• Valid driver's license in good standing required.

Responsabilités

• Build strong relationships with individuals served, families, host home providers, and direct care workers.
• Champion individuals' goals and ensure their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure positive matches.
• Collaborate to create and update Individual Support Plans in line with HCBS regulations.
• Oversee service delivery and contract compliance through home visits and fieldwork.
• Provide on-call support to providers and direct care workers as needed.

Status:Salaried, Days with OnCall to support case load (crisis intervention), Travel for home visits (can be up to 2 hours) agency vehicle or mileage reimbursement available.

• Relationship Builder:You’ll monitor the relationships between individuals and their home providers, ensuring the pairing is a perfect, positive, and supportive match.
• Architect Personalized Plans:Collaborate with our service planning team to create Individual Support Plans. You’ll weave together a person's unique wishes with the necessary policy and regulatory requirements.
• Keep it Real in the Field:Oversee service delivery and contract compliance by hanging out where the magic happens. You’ll conduct regular home and service environment visits.
• Be the Safety Net:Provide on-call support to our wonderful providers and direct care workers when they need a helping hand or a quick problem solved.

What You’ll Bring to the Table:

• The Degrees & Deeds:A Bachelor’s degree in human services or related field ispreferred, along with a minimum of two years of related work experience.

• The Credentials:A valid driver's license in good standing (you've gotta do those home visits!).
