Job summary

Gurwin Certified Home Care Agency is seeking a Physical Therapist to cover Nassau County for home health visits on a fee-for-service basis. The position requires a NYS PT license, PT program completion, and at least one year of experience, with home health preferred.

Ability to travel to patients' homes and work with laptop/tablet is expected. We offer competitive compensation per visit: $85 for initial visit and $80 for follow-up visits, plus mileage reimbursement and a supportive,

Qualifications

• A NYS Physical Therapy license is required.
• Graduate from a PT program approved by ABPT or AMA.
• Minimum of one year experience as a physical therapist.
• Home health experience preferred.
• Comfortable with laptop/tablet for home visits.

Responsibilities

• Reports initial status, changes, therapy plans/goals, ongoing status, and discharge plans.
• Provides timely documentation including assessments, plans, goals, visit data and billing.
• Instructs patients, families and team members to support care and follow-up.
• Evaluates social/architectural environment; makes feasible recommendations.
• Complies with regulatory criteria.
• Notifies agency of visit schedule and changes.
• Provides leadership and clinical guidance to team members.
• Attends regular continuing education programs.
• Performs related duties as necessary.
• Adheres to customer service standards: empathy, courtesy and respect.

New York State Physical Therapy License
