Job summary

Community Services Group in State College, PA is seeking a Mobile Psychiatric Rehabilitation Associate on a PRN/zero scheduled hours basis. The role supports individuals in achieving recovery goals, conducting assessments, and implementing individual support plans within our Adult Mental Health Mobile Rehabilitation Program.

The position requires either a Bachelor’s degree or an Associate degree with one year of mental health experience, plus a valid driver’s license to accompany individuals in

Qualifications

• Bachelor's degree or Associate degree with 1 year mental health experience.
• Must have a valid driver’s license and be able to drive a passenger van and accompany individuals into the community.
• Ability to interact with individuals in the community while maintaining rights and dignity.

Responsibilities

• Coordinate and complete initial and ongoing assessments of individuals.
• Coordinate, review, and implement individual support plans.
• Assess, evaluate, and document individual performance.
• Identify abilities, strengths, and assets and help individuals use them to achieve goals.
• Implement rehabilitation team decisions in all program settings.
• Facilitate group processes and activities within the program.
• Transport individuals to community-based activities.
• Model trauma-informed care principles: safety, choice, collaboration, trustworthiness, empowerment.
• Promote cultural, racial, language and gender access, diversity, equity and inclusion.
• Adhere to PRS Code of Ethics and rights statements.

This position meets the regulation requirements of a Psychiatric Rehabilitation Worker. The Psychiatric Rehabilitation Associate reports directly to the Program Director. This position is part of our Adult Mental Health (MH) Services Mobile Psychiatric Rehabilitation Program.
