Mobile Professional Care Manager - Blair County

UPMC Community Care Behavioral Health is seeking a full-time Mobile Professional Care Manager to support the Community Services Department in Blair County.

The Mobile Professional Care Manager will work flexible hours, Monday through Friday, with travel throughout Blair County to meet with members within the community. This role is primarily community-based, with most time spent traveling and some work completed remotely/from home.

The care manager will work directly with members with physical and behavioral health needs and/or substance use disorders who have unstable, inadequate, or no residence. Intensive care management is provided in collaboration with local housing partners.

The role involves conducting assessments, care coordination, facilitating communication among providers, and developing care plans tailored to individual needs, including behavioral, clinical, social, and environmental factors.

Responsibilities include:

  1. Conduct face-to-face assessments in community settings or facilities, including hospital discharge coordination.
  2. Collaborate with providers for assessments, treatment, and aftercare planning.
  3. Identify member concerns and needs across behavioral, clinical, social, and environmental domains.
  4. Link members and families to primary care, behavioral health, and social services.
  5. Manage cases within departmental standards and participate in meetings and treatment planning sessions.
  6. Make referrals and provide community resource expertise.
  7. Assess member knowledge and provide education as needed.
  8. Implement clinical interventions to achieve optimal outcomes.
  9. Develop outreach plans for members with irregular contact to prevent crises and recidivism.
  10. Respond to crisis calls and coordinate care across providers and services.
  11. Identify barriers to care and develop integrated care plans.
  12. Refer members to community-based services and suggest improvements for provider issues.

Minimum qualifications include:

  • PA licensure in health or human services field and a master's degree, or licensed RN (BSN preferred), or Paramedic/EMT with 6-8 years of experience. Preference for RN or human services licensure.
  • At least three years of experience in behavioral health, utilization management, home care, discharge planning, or case management.
  • Behavioral health experience is a plus.
  • Five years of community-based case management experience preferred.
  • Knowledge of best practices with special needs populations and excellent organizational and communication skills.
  • Proficiency in Microsoft Office and ability to learn new software.
  • Experience in managed care and with geriatric populations is preferred.
  • Certification in substance use disorders is helpful.

Licensure and certifications required:

  • CPR (AHA standards), within 30 days of hire.
  • Driver's license, FBI clearance, and Act 33, 34, 73 clearances.
  • Current licensure in applicable health or human services fields.
