Job summary

Access Healthcare Associates is hiring a Mobile Primary Care Nurse Practitioner or Physician Assistant to bring high-quality geriatrics to homes and senior living communities in Daly City and surrounding areas.

The role emphasizes autonomy with support, comprehensive assessments, chronic care management, and collaboration with palliative, psych, and neuro teams. A valid NP/PA license and 1–2 years of geriatrics or primary care experience are required.

Qualifications

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.
