Atlas Medical in Lincoln, NE is seeking a Full-Time Nurse Practitioner or Physician Assistant to deliver mobile on-site primary care to residents in assisted living and senior communities. You’ll manage complex cases autonomously and educate patients and families on self-care and disease management.

Qualifications

• Minimum of one year of primary care experience.
• Valid NP or PA licensure in state of practice.
• Ability and desire to work independently and travel between communities.
• Proficiency with an EMR system (Elation).
• Excellent verbal and written communication and time management.
• Criminal background clearance, CPR certification, and TB test.

Responsibilities

• Deliver mobile on-site primary care to residents in assisted living and senior communities.
• Autonomously manage complex healthcare scenarios with multiple comorbidities.
• Educate patients and families on self-care, disease management, and end-of-life discussions.
• Collaborate with facility staff and Atlas Patient Care support teams.
• Maintain confidentiality and uphold high quality measures.
