Job summary

Qualifications

• Active Maryland Physical Therapist (PT) license – Required.
• License allowing practice in Maryland and Washington, D.C. – Preferred.
• Minimum of 6 months of physical therapy experience (home health preferred).
• Minimum of a Bachelor's, Master's, or DPT in Physical Therapy from an accredited program.
• Strong clinical assessment and rehabilitation skills.
• Excellent verbal and written communication skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Reliable transportation for home visits (mileage reimbursement provided).
• Proficiency with computers, tablets, or iPads for documentation and communication.

Responsibilities

• Conduct comprehensive physical therapy evaluations and assess patients' mobility, strength, balance, and rehabilitation needs.
• Develop individualized treatment plans based on each patient's condition, goals, and physician orders.
• Implement evidence-based therapeutic interventions to improve mobility and independence.
• Educate patients, family members, and caregivers on exercises, mobility techniques, fall prevention, and home safety.
• Monitor patient progress and document treatment outcomes, modifying care plans as needed.
• Communicate patient progress and changes to physicians, family, and healthcare team.
• Maintain accurate, timely, and compliant clinical documentation.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality patient care.
