Job summary

Hospital to Home LLC is seeking a skilled Paramedic to perform advanced EMS care in the field and during transport, bridging hospital discharge to home health. You’ll play a key role in safe, efficient, and clinically appropriate medical transport.

You will respond to calls, provide ALS care, coordinate with RNs and case managers, monitor patient condition, and ensure complete documentation while upholding confidentiality and professional standards.

Qualifications

• Current State Paramedic License (and NREMT if required by state).
• Current certifications: BLS, ACLS, PALS; ITLS/PHTLS preferred.
• Valid driver's license with clean record and ability to lift 125 lbs.

Responsibilities

• Respond to calls for medical assistance and transports.
• Provide ALS care including medication administration and airway management.
• Collaborate with RNs, case managers, and other professionals for patient transitions.
• Document patient condition and care during transport; ensure confidentiality.
