Job summary

Clare Medical of New Jersey seeks an APN/PA to provide housecalls across Northern Mercer and Somerset County. You will perform detailed patient assessments, wellness and sick visits, manage chronic diseases, and collaborate with other providers.

Candidates must have an unrestricted NJ APN license, DEA/CDS, at least 1 year of clinical experience, and be willing to travel regionally. Excellent salary with bonus opportunities and travel reimbursement, plus a supportive team and onboarding.

Qualifications

• Unrestricted APN license in New Jersey.
• At least 1 year of clinical experience.
• Current DEA/CDS license.
• Willingness to travel within the region.
• Bi-lingual a plus.

Responsibilities

• Initiate and complete detailed patient assessments and physical exams.
• Wellness exams and sick visits.
• Evaluate and manage common and chronic diseases.
• Order, interpret and evaluate diagnostic and lab results.
• Refer and collaborate with other providers.
• Prescribing pharmacological and non-pharmacological therapies.
