Job description

Job Summary

Improve the overall health of the communities we serve by enhancing access to quality health, wellness, and supportive services. This role supports population health initiatives, case management, and health equity efforts while providing direct patient care.

Essential Duties and Responsibilities
  • Perform independent visits under standing order procedures for patients outside the clinic, including health assessments, medication reconciliation, wound care, lab work, immunizations, and patient education.
  • Collaborate with mobile medicine, outreach, and primary care teams to support population health initiatives.
  • Work with the health equity team to identify and implement health outcome initiatives that reduce disparities for marginalized and excluded patient populations.
  • Lead case management efforts, coordinate care, and create care plans for unengaged patients to improve chronic conditions and overall health outcomes.
  • Provide patient, family, and caregiver education, including motivational interviewing and patient‑centered health action plans.
  • Collaborate with internal and community resources to address patients’ social determinants of health needs.
  • Perform other duties as assigned in support of the CHAS Health Mission and Core Values.
Qualifications
  • Valid Washington State Registered Nurse license.
  • Bachelor of Science in Nursing from a four‑year college or university (preferred).
  • Minimum of three years’ experience in a clinical setting as a Registered Nurse (preferred).
  • Experience in triage and in community health settings is a plus.
  • Strong familiarity with electronic health record systems (preferred).
  • Experience with process improvement initiatives (plus).
  • Excellent customer service, oral and written communication skills.
  • Compassion for patients and a commitment to a safe and respectful environment.
  • Ability to work well in a team and multitask in a fast‑paced environment.
  • Computer proficiency and basic technology troubleshooting skills.
  • Basic Life Support (BLS) CPR/AED certification required.
Physical Demands

The employee must stand, sit, and be mobile one‑third to two‑thirds of the time. Hands are used for various tasks over two‑thirds of the time, with reaching, climbing, or balancing less than one‑third of the time. Lifting up to 26 pounds occasionally; the majority of lifting is under 10 pounds. Audible communication is required for most of the day.

Compensation & Benefits
  • Competitive salary range: $38.03 – $57.52 per hour.
  • Annual milestone bonus of $500 for each year worked, up to $5,000.
