Job summary

The University of Vermont Health Network is seeking a Community Paramedic to join the Mobile Integrated Health program serving central and northeast Vermont. You will conduct patient assessments, provide education, assist with chronic disease management, medication reconciliation, and post-hospital follow-up.

Working within an interdisciplinary team, you’ll help patients navigate care and resources, perform outreach, and support home-visiting models across the region.

Qualifications

• High School Diploma or GED required.
• Associate's or Bachelor's Degree preferred.
• Vermont EMS Paramedic Certification required.
• NREMT-P Paramedic Certification required.
• New York Paramedic Certification preferred.
• CP-C (Community Paramedic Certification) preferred after 12–24 months.
• 3+ years direct patient care experience in 911/critical care/community paramedicine.

Responsibilities

• Provide patient-centered care in homes, community settings, and non-traditional environments.
• Conduct assessments and post-hospital follow-up; assist with chronic disease management and medication reconciliation.

The Community Paramedic is a key member of an interdisciplinary healthcare team providing patient-centered care in homes, community settings, and other non-traditional care environments throughout central and northeast Vermont. This role supports patients across the continuum of care by conducting assessments, providing education, assisting with chronic disease management, medication reconciliation, post-hospital follow-up, and care coordination.

Working closely with healthcare providers, behavioral health specialists, case managers, social service agencies, and community partners, the Community Paramedic helps patients navigate healthcare and community resources while improving access to care, reducing avoidable emergency department visits and hospital readmissions, and decreasing inappropriate utilization of 911 services.

The Community Paramedic will also perform outreach visits into rural communities and support home-visitation service models throughout the region.

Project Description

This position supports a hospital-anchored Mobile Integrated Health (MIH) program that deploys an interdisciplinary care team throughout a multi-county service area in central and northeast Vermont. The program delivers healthcare services directly within communities to improve access to care and address both medical and social needs.

Additional services provided through the program include:

• Primary, preventive, and transitional care services, including wound care, immunizations, and post-hospital follow-up.
• Behavioral health and social support services, including behavioral health screening and brief intervention, substance use disorder services with medication-assisted treatment support, and health-related social needs navigation.

• Associate's or Bachelor's Degree preferred
• Vermont Office of Emergency Medical Services Paramedic Certification (required)
• National Registry Emergency Medical Technician Paramedic (NREMT-P) (required)
• New York Emergency Medical Services Paramedic Certification (preferred)
• CP-C (Community Paramedic Certification) preferred after 12-to-24 months of hire
• 3 or more years of direct patient care experience in 911, critical care, or community paramedicine.
