Advocate Health is expanding its Hospital at Home Program in Park Ridge, IL, offering full-time RN positions delivering hospital-level care in patients' homes. The role blends autonomy with an interdisciplinary team approach in a home setting, requiring strong clinical skills and independent work ethic.

The RN will manage 3–4 patient assignments per 12-hour shift, including possible admits and discharges, with travel within the Lutheran General Hospital service area and a 9a–9p schedule.

Qualificações

• Active RN license in Illinois.
• BLS certification (AHA) required.
• 2+ years of acute inpatient care or ED experience preferred.
• Valid driver's license with clean driving record.
• Ability to work independently in home settings.

Responsabilidades

• Deliver acute, hospital-level care in patients' homes.
• Perform skilled clinical tasks (IV starts, phlebotomy, medication administration).
• Monitor patient conditions and escalate changes as needed.
• Collaborate with a virtual and in-person interdisciplinary team.
• Use evidence-based practice to ensure safe, patient-centered care.
• Coordinate discharge planning and ongoing care needs.
• Promote a culture of safety and high-quality outcomes.

What We're Looking For

Required:

• ActiveRN license in the state of Illinois
• BLS certification (AHA)

• Ability to workindependently in home settings
• Excellent communication and critical thinking
• Comfortable withtechnology and remote monitoring tools
