Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work independently and as part of a multidisciplinary team in home/community settings.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility.
• Monitor and document vital signs, observations, and changes in condition.
• Communicate with the healthcare team regarding patient status and needs.
