Job summary
Remedy Home Health Care in East LA County is seeking a licensed Physical Therapist to join our home health team in Montebello, Downey, and Bellflower. This Per Diem / PRN role offers flexibility, autonomy, and the chance to provide high‑quality rehab and community‑based care directly to patients in their homes.
As a Home Health PT, you’ll work with senior and geriatric patients recovering from illness, injury, or surgery, delivering skilled therapy in home health, assisted living, and community
Qualifications
- Licensed Physical Therapist (PT) in California required.
- CPR certification (AHA approved).
- Minimum 1 year of clinical experience; home health preferred.
- Experience with geriatrics, senior care, rehab, or community-based care preferred.
- Strong clinical, communication, and patient education skills.
- Valid driver’s license and reliable transportation.
Responsibilities
- Conduct in-home PT evaluations and reassessments.
- Develop and implement personalized rehab care plans.
- Improve strength, balance, mobility, and functional outcomes.
- Provide therapy in home health, assisted living, and community care settings.
- Educate patients and families on fall prevention and safe mobility.
- Document visits using EMR (WellSky/Kinnser).
- Collaborate with interdisciplinary home health teams.
