Job summary
Remedy Home Health Care in Los Angeles County is seeking a licensed Physical Therapist (PT) to join our Home Health team serving Huntington Park, Maywood, South Gate and surrounding areas. This Per Diem/PT role offers per visit pay and the flexibility to work part-time, PRN, or full-time.
As a Home Health PT, you will deliver skilled therapy to senior and geriatric patients in home health, assisted living, and community care settings, focusing on fall prevention, mobility, and independence.
Qualifications
- Licensed Physical Therapist (PT) – California (Required)
- CPR Certification (AHA approved)
- Minimum 1 year of clinical experience (Home Health preferred)
- Experience in senior care, geriatrics, rehab, or community-based care preferred
- Strong clinical, communication, and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
Responsibilities
- Conduct in-home physical therapy evaluations and reassessments
- Develop and implement personalized rehabilitation care plans
- Improve strength, balance, mobility, and functional outcomes
- Provide therapy in home health, assisted living, and community care settings
- Educate patients and families on fall prevention and safe mobility techniques
- Document visits using WellSky (Kinnser) EMR
- Collaborate with interdisciplinary home health teams
