Job summary

Sweetwater Care is seeking a Home Health Certified Occupational Therapy Assistant to support patients in their homes across our service area. This role requires an associate degree and an active license to practice, along with CPR certification and a valid driver’s license.

The COTA will travel to client locations, perform skilled OT interventions, document progress, and collaborate with a multidisciplinary team to ensure safe, effective home health care delivery.

Qualifications

• Valid license, registration, certification or permit to practice in the states of agency operation
• Must be a licensed driver who can travel to all business locations
• Must possess and maintain valid CPR certification while employed in a clinical role

Education

Associate degree from an accredited school of Occupational Therapy
