Qualifications

• Current OT license for the state where services are rendered.
• CPR certification, valid driver's license, vehicle insurance, and access to reliable transportation.
• Ability to reliably commute to multiple patient residences within the geographic area.

Responsibilities

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Develop therapy plans of care in coordination with physicians and the health care team.
• Document patient progress and communicate changes to supervising nurse and physician.
• Conduct patient assessments using functional ability tests.
