Humana in Illinois is seeking a Field Care Manager Registered Nurse to identify barriers, monitor care, and create plans for Medicaid members. The role requires 75-90% travel within Illinois and collaboration with stakeholders.

The position is hybrid–home based with some onsite work at the Schaumburg market office, offering 40 hours per week and a focus on community health support and EMR usage.

Qualificações

• Residency in IL within listed ZIP codes or 10-mile radius.
• Illinois RN license without disciplinary action.
• Clinical nursing experience in hospital, acute care, or home health.
• Ability to travel 75-90% within Illinois.
• Knowledge of community health and social service agencies.
• Ability to use EMR and related electronic information systems.

Responsabilidades

• Identifies and resolves barriers that hinder effective care.
• Ensures member progress toward outcomes via ongoing assessments/evaluations.
• Create plans of care.
• Communicates with internal and external stakeholders.
• Understands department, segment, and organizational strategy and objectives.
• Follows established guidelines/procedures.
• Visits Medicaid members in homes, supportive living facilities, and long-term care facilities with 75-90% local travel.
