Job summary

Care2U is seeking experienced Nurse Practitioners and Physician Assistants to provide in-home urgent and acute care in Manhattan, the surrounding boroughs, Westchester and Nassau counties, collaborating with Telehealth Emergency Physicians.

You will conduct in-person visits, manage acute illnesses, document in the EHR, and work with a multidisciplinary team to deliver high-quality, patient-centered care in the home.

Qualifications

• Master's Degree in PA or NP Studies from an accredited university.
• Valid NP or PA Certification and license to practice in New York.
• DEA certificate and BLS certification.
• Experience in providing care in an emergency department or hospitalist medicine is a plus.

Responsibilities

• Conduct in-person home visits to manage acute illnesses and injuries.
• Provide treatment and clinical management per evidence-based guidelines.
• Collaborate with a multidisciplinary team of field providers and care coordinators.
• Assess patients for risk factors related to acute and chronic illness care.
• Maintain accurate patient records in the EHR meeting compliance standards.
• Respond to on-demand and scheduled calls focusing on quality and efficiency.
