Job summary

Catholic Charities of the Archdiocese of Newark is seeking a Full Time Case Manager for the Mobile Response & Stabilization Services Program in Jersey City, NJ. You will deliver in-home crisis intervention, safety planning, stabilization, and comprehensive case management to families and children in crisis.

Requirements include a Bachelor’s degree in social work or related field and a valid driver's license.

Qualifications

• Bachelor Degree in social work or a related field.
• Valid driver’s license.
• Bilingual fluency in English and Spanish preferred but not required.

Responsibilities

• Provide in-home and community based crisis de-escalation/intervention and assessment, safety planning as well as stabilization and case management services to families and children according to the families’ individualized service needs.
• Coordination of discharge/transition services to include referral and linkages to services and supports appropriate to the level of need of the child/family.
