The role emphasizes rapid assessment, safety planning, and connecting clients with appropriate services. The position requires flexible scheduling, including nights and weekends, and may involve working beyond a 40-hour week during emergencies.

Responsibilities

• Provide short term mobile, on-site crisis intervention to individuals in behavioral health crisis to assess, treat, and stabilize situations and reduce risk.
• Offer brief solution-focused interventions and reassess current level of need for adults awaiting higher level of care.
• Conduct post-crisis evaluation and follow-up during the interim period when applicable.
• Collaborate with support staff to provide resources, referrals, and psychoeducation to families.
• Attend community-based meetings with the individual, family, and providers to advocate and address safety concerns.
• Complete collateral contacts with the individual’s providers and document findings.
