Job summary

Empower Fitness PT in Boca Raton, FL is seeking a full‑time Physical Therapist for Concierge Mobile Care to provide in‑home and community based services. You will perform comprehensive evaluations, create individualized treatment plans, and deliver hands‑on therapy focused on rehabilitation, pain management, and functional improvement.

Coordinate with physicians, educate clients, and document visits while managing a mobile schedule and equipment.

Qualifications

Strong foundation in Medicine and Physical Therapy with evidence‑based practice in mobile/concierge setting.
Experience in Rehabilitation, designing and progressing individualized treatment plans for orthopedic, neurological, or sports conditions.
Excellence in Patient Care with client‑centered approach, clear communication, safety, outcomes and satisfaction.
Background in Home Care, delivering services in non‑clinical environments and adapting interventions to home/community settings.

Responsibilities

Perform comprehensive evaluations for mobile clients.
Develop individualized treatment plans and progressions.
Deliver hands‑on physical therapy focused on rehabilitation, pain management, and functional improvement.
Coordinate care with physicians and other health professionals.
Educate clients and caregivers on exercises and injury prevention.
Document all visits in accordance with professional and regulatory standards.
Manage a mobile schedule, maintaining equipment and supplies.
Contribute to a positive client experience aligned with concierge care model.
