SCAN Health Plan is seeking an LVN/LPN to deliver direct, onsite and remote care to Embrace members in assisted living and community settings under NP/MD supervision.
You will conduct focused assessments, administer vaccines, and support care transitions as part of a multidisciplinary team. This field-based role emphasizes quality, collaboration, and member wellbeing in Oregon.

The LVN/LPN provides direct, onsite and remote care to Embrace members residing in assisted living and community-based settings under the direction of Nurse Practitioners and Physicians. This role focuses on focused assessments, wound care, vaccination support, IV therapy within scope, catheter care, and strong collaboration with community staff. The LVN serves as a visible, professional brand ambassador and clinical extension of the primary care team, supporting high-quality care delivery and reinforcing Embrace workflows within assigned communities.
