SCAN Health Plan is seeking an LVN/LPN to deliver direct on-site and remote care to Embrace members in Portland, OR. Under NP/MD direction, you will perform focused assessments, wound and catheter care, vaccinations, and IV therapy within scope to support high-quality care delivery.

The LVN/LPN provides direct, onsite and remote care to Embrace members residing in assisted living and community-based settings under the direction of Nurse Practitioners and Physicians. This role focuses on focused assessments, wound care, vaccination support, IV therapy within scope, catheter care, and strong collaboration with community staff. The LVN serves as a visible, professional brand ambassador and clinical extension of the primary care team, supporting high-quality care delivery and reinforcing Embrace workflows within assigned communities.
All care is delivered under NP/MD direction and within applicable state and organizational regulations.
