Job summary

InterCommunity, Inc. in Connecticut is hiring a Registered Nurse to join a mission-driven team. The RN will work under the Clinical Practice Manager, providing evaluation, assessment, and direct patient care across primary care and psychiatry.

The role includes triage, vital signs, medication administration, patient education, and collaboration with a multidisciplinary team to ensure quality care. CT RN license and BLS/CPR are required; EPIC is preferred.

Qualifications

• Graduation from an accredited nursing program.
• Current State of Connecticut RN licensure required.
• 2 years of community health, addiction medicine and/or psychiatric nursing required.
• BLS/CPR certification required.
• EPIC experience preferred.

Responsibilities

• Provides direct patient care and carries out orders as directed by the medical provider.
• Conducts nursing visits and education groups as ordered.
• Maintains accurate, detailed reports and records.
• Monitors and reports symptoms or changes in patients' conditions; assess medication efficacy.
• Ensures gaps in care for screenings and chronic care tests are completed on schedule.
• Performs crisis intervention and safety assessments as needed.
• Coordinates with the healthcare team to implement or evaluate care plans.
