Continuum Therapy Partners in Milwaukee, WI seeks a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a licensed PT. You will help implement treatment plans and monitor patient progress while ensuring high-quality care.

The ideal candidate holds an active PTA license, an associate degree, and strong communication to educate patients and families. This full-time daytime role offers benefits within a growing home-health network.

Formación

• Associate degree in PTA program.
• Current, unencumbered state PTA license.
• 1 year of PTA experience preferred.
• Implements treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
