Job description

Primary Care Solutions provides industry-leading Primary Care services to Veterans. Veteran-led and clinician-managed, we deliver compassionate, expert medical care, establishing personal bonds with our Veterans in our Community-Based Outpatient Clinics (CBOC’s). We have consistently achieved high marks from the VA in our CBOC’s, and we are excited to discover first-rate colleagues to join our group.

RESPONSIBILITIES

  • Provide a friendly, welcoming, and confidential environment for our Veterans.
  • Involves specialized administrative judgment and flexible use of various clinical flow processes.
  • Recommend changes to existing clinic procedures based on current administrative guidelines.
  • Utilize numerous advanced patient systems in support of multiple clinics involved in an interdisciplinary coordinated care delivery model.
  • Coordinate with the patient care team to review clinic appointment availability to ensure effective support of clinic needs.
  • Develop and maintain effective communication with patients, interdisciplinary teams, VA medical centers, and other agencies.
  • Process incoming patient secure messaging through MyHealthyVet and coordinate with care team as appropriate.
  • Participate in team huddles and meetings to manage and follow-up with patient care.
  • Enter appropriate information into the electronic record.
  • Monitor pre-appointment information to assure readiness for patient visits/procedures.
  • Manage patient systems to verify accuracy and resolve issues regarding appointments.

JOB REQUIREMENTS:

  • High School Diploma/GED
  • Knowledge of computer systems including the MS Office Suite
  • Demonstrated high-quality customer service and organizational skills
  • Basic Life Support certification from The American Heart Association (to be renewed annually)
