Job summary

Socket.dev is seeking a behavioral health case manager to support program goals by coordinating access to medical, educational, social, vocational, and housing resources. The role involves crisis intervention, referrals, and whole-person care coordination in California.

Required qualifications include a Master’s degree in a behavioral science and active BBS licensure (MFT/LCSW/LPCC), plus a CA driver's license.

Qualifications

• Master’s degree in a behavioral science with valid BBS registration or license (MFT, LCSW, LPCC) required.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Experience providing mental health and/or substance use disorder services preferred.
• Cultural competency understanding required.
• Bilingual preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Support client in court, as needed, and collaborate with CARE team providers.
