Job summary

NYC Health and Hospitals is seeking a MetroPlus Care Coordinator Level I in New York, NY to coordinate and monitor patient-centered quality care. Candidates should have a nursing baccalaureate and at least two years of RN experience, plus NYS RN licensure.

Qualifications

  • Bachelor's degree in Nursing or related health care field.
  • 2 years of experience as a Registered Professional Nurse.
  • Valid New York State license and current registration to practice as an RN.

Responsibilities

  • Review each patient’s chart to ensure documentation supports the plan of care.
  • Coordinate and facilitate timely implementation of discharge plans.
  • Participate in multidisciplinary rounds and review the plan of care.
  • Collaborate with healthcare professionals to identify and eliminate barriers to care.
  • Communicate with reimbursement departments and third-party payers for authorizations.
  • Plan strategies to reduce length of stay and achieve positive patient outcomes.
  • Initiate discharge planning by assessing patient and family needs.
  • Perform post Emergency Department discharge follow-up calls to patients.
  • Maintain effective communication with healthcare teams regarding discharge planning.
  • Engage with community organizations to assist in patient care transitions.

About This Role

The MetroPlus Care Coordinator Level I is responsible for coordinating and monitoring patient-centered quality care, ensuring optimal resource utilization and compliance with regulations. This role is ideal for experienced nurses who are skilled in discharge planning and patient advocacy.

  • Schedule: Full-time, 7:00 A.M – 3:30 P.M
What You'll Do

Requirements

Education:
