Job summary

Mission Community Hospital is seeking a Mental Health Worker to support licensed nurses in delivering patient care in a mental health setting. You will monitor vital signs, assist with ADLs, and communicate changes in patient status to the RN.

The role emphasizes de‑escalation and safety, with focus on timely documentation and compassionate care. The position requires flexible scheduling, teamwork, and the ability to respond quickly to changing patient conditions.

Qualifications

• Ability to communicate clearly with patients, families and staff.
• Current BLS/CPR certification or ability to obtain within 60 days.
• Basic understanding of medical terminology and patient care workflows.
• Experience assisting with activities of daily living and vital signs.
• Ability to lift or transfer patients with proper support or assistive devices.

Responsibilities

• Assist licensed nurses with patient care under supervision.
• Document vital signs, intake/output, and report changes to RN promptly.
• Support de-escalation and safety procedures per hospital policy.
• Aid in admissions tasks, orientation, and organization of patient belongings.
• Assist with ADLs including bathing, grooming, mobility, and nutrition monitoring.
• Recognize patients at risk for falls and follow fall prevention protocols.
• Provide care focusing on psychosocial and physical needs under RN direction.

Job description

The MHW assists the licensed nurses in providing patient care in the mental healthcare setting. He/she communicates with the Administrator, Manager, Charge RN and co-workers as appropriate about changes in patient’s clinical condition including vital signs, behavioral and physical symptomatology. The BHU MHW is able to respond quickly to changes in patient’s status. He/She is able to perform basic nursing functions under direction of Charge nurse.

Responsibilities

• Insures that all Patient Rights are followed and adhered to.
• Participates in providing cost-effective, quality, efficient patient care, under the direction of licensed nursing personnel in a timely manner.
• Demonstrates leadership skills in de-escalation, seclusion, or restraint episodes following all hospital policies and procedures.
• Obtains and documents, vital signs, height, weight, intake, and output accurately and reports all findings to nursing personnel. Consistently alerts RN to changes in vital signs, inadequate intake, or changes in the patient’s condition.
• Completes admission tasks as required by unit including, cataloging of patient’s belongings and assisting RN in providing new patient orientation to unit/schedule. Labels patient’s belongings that are stored and follows hospital policy for safeguarding valuables.
• Recognizes and assists patients with ADL needs including bathing, ambulation, grooming and monitoring dietary intake taking into account the patients age and physical condition.
• Assists the RN in identifying patients at risk for falls and follows fall precaution policies.
• Provides care under the direction of a Registered Nurse to patients, including actions that meet their psychosocial, age specific, and physical needs.
• Demonstrates basic knowledge of mental health disorders.

Qualifications

Required

• Able to perform routine patient-care activities; physical strengths to assist patients in ambulating, turning in bed, and transferring in and out of bed or wheelchair.
• Good communication skills. Must be able to read, write, speak, and understand English
• Able to perform simple arithmetic calculations.
• Ability to understand and use medical terminology.
• Current BLS/CPR
• FIRE Card obtained within 60 days of employment
• Initial MABPRO MAB or current MABPRO certification by completion of unit orientation
• Advanced MABPRO certification by completion of unit orientation

Preferred

• Six months as a Mental Health Worker, Certified Nursing Assistant, Patient Care Technician, Medical Assistant, Emergency Medical Technician or Paramedic preferred.
• Minimum of six months adult Behavioral Health preferred, Acute Care, Long Term or sub- acute direct patient care experience within the last two years preferred; or a minimum of six months experience providing direct care, monitoring, and/or evaluation of individuals diagnosed with mental illness/disability or a medical condition
