Job Description: Provides ongoing mental health and psychosocial client services, including screening, assessing client needs, and developing treatment plans. Participates in community development initiatives and PHC evaluation. Promotes quality healthcare outcomes within established standards to ensure high standards of primary health care, aligned with the organization’s mission, policies, and procedures.

Education: Bachelor of Science in Nursing (BScN) or equivalent in relevant health-related fields such as Social Work, Physical Therapy, Chemical Dependency, Occupational Therapy, Psychiatric Nursing, or Education.

Additional Requirements: Preference for HSAS members; registered or eligible for registration with the appropriate licensing body; adherence to SAHO/HSAS Collective Agreement.
