Job summary

Vitae Health Systems is hiring a Psychotherapist to provide mental health services in a skilled nursing facility. You will assess, diagnose, and treat residents, offering individual, group, and family therapy to support well-being and adjustment to illness and life changes.

The role requires a Master’s degree and an unrestricted state license (IL). You will work with an interdisciplinary team, maintain HIPAA-compliant documentation, and participate in care planning.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted state license (e.g., LCSW, LCPC, LMFT, or Licensed Psychologist)
• 2+ years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
• Strong clinical assessment, diagnostic, and treatment planning skills
• Knowledge of mental health conditions common in SNF populations (depression, anxiety, dementia)
• Proficiency in evidence-based therapeutic approaches (CBT, trauma-informed care, grief counseling)
• Ability to work collaboratively within an interdisciplinary team
• Excellent communication and interpersonal skills with residents, families, and staff
• Familiarity with electronic health records (EHR) systems preferred

COMPENSATION: The hiring range for this position is $70,000-$90,000, with the opportunity to earn additional compensation through bonuses; candidates may also choose between a base salary structure or a variable compensation plan (commission), which may provide higher earning potential

Position Summary:

Vitae Healthis seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed

• Develop, implement, and monitorindividualized treatment plans

• Provide individual, group, and/or family therapy sessions
