evolvedMD in Colorado is seeking a Mental Health Therapist – Professional Counselor – Licensed Social Worker – Marriage and Family Therapist to join our collaborative team. You’ll work with primary care partners to deliver whole-patient care in a dynamic behavioral health program.

Qualifications

• Master’s degree in a behavioral health field and an active license (LAC, LPC, LMSW, LCSW, LAMFT, or LMFT) in good standing.
• Proven dedication to patient impact and clinical outcomes with experience as counselor, therapist, or social worker.
• Ability to collaborate with diverse health providers to deliver Whole Patient Care.

The Basics

• Location: Split Between Two Clinics: Mon & Fri Fort Collins, CO 80525Tues to Thurs Fort Collins, CO 80526
• Salary + Bonus Potential: $65,000 - $89,400 annually (depending on experience, market, and bonus opportunities)
• Reporting directly to the Clinical Manager

• Partner with primary care providers to deliver proven whole-patient care.
• Provide brief, structured intervention techniques (e.g. Behavioral Activation, Motivational Interviewing, Solution Focused) and evidence-based psychosocial treatments (e.g. Cognitive-Behavioral Therapy, Interpersonal Therapy).
• Leverage real time data to drive positive patient outcomes, immediately understanding your impact on the patients you serve.
• Positively contributes to an ever-expanding clinical team all committed to a new, innovative way to serve patients.

The Right Fit

Our most successful clinicians possess:

• Minimum Requirements – Master’s degree in a behavioral health field (social work, counseling, marriage and family therapy, etc.) along with an active license (LAC, LPC, LMSW, LCSW, LAMFT, or LMFT) in good standing as ascribed by your state and governing body.
• Proven + Passionate Drive – A love for and commitment to patient impact while also driving clinical outcomes as demonstrated by previous experience as a counselor, therapist, or social worker with patients who experience diverse issues.

• Competitive Compensation Package – We offer competitive benefits including medical, dental, vision and EAP for any additional needs. 200+ hours of PTO / Holiday per anniversary year which increases with tenure. For those in the military reserves we offer 80 hours of paid military leave, and after one year of employment we offer 80 hours of Parental Leave for each birth/adoption. Salary Range: $65,000 - $91,400 annually (depending on experience, market, and bonus opportunities).
• Free to You Clinical Elevation – The road to independent licensure (LPC, LCSW, and LMFT) is simply part of your daily plan. You will receive no cost clinical supervision (for LAC, LMSW, LAMFT) built in your workday.

The investment doesn’t stop with learning as you will receive an automatic $5K salary increase after year one of participating in leadership program with an investment up to $10,000 following program completion after year two.
