Job summary

Sevita in Peoria, Arizona is looking for dedicated mentors to assist individuals with intellectual and developmental disabilities. Qualifications

  • Able to obtain a level one fingerprint card.

Responsibilities

  • Assist individuals with daily activities including personal care and socialization.
  • Document progress and review service plans.

Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. Location & Schedule: Jasmine Group Home (located near 83rd Ave & Cactus). Full Time, Tuesday‑Saturday 3p‑11p. Pay Rate: $16 per hour.

Mission and Responsibilities
  • Provide assistance with daily activities including socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
  • Accurately document progress and activity; review records and logs to stay up‑to‑date with changes in service plans.
  • Provide clinical expertise, which may include knowledge for specialized populations such as intellectual and developmental disabilities, brain injury, emotional or behavioral disability, or expertise in special disciplines such as behavioral support, early intervention, crisis intervention, or others.
  • May accompany individuals to medical appointments; relay orders and information to and from medical providers. Travel between sites, to individuals’ homes, and may accompany them into the community.
Qualifications
  • High school diploma or equivalent required.

  • States may have additional requirements.
  • Strong attention to detail, organizational skills, and the ability to multi‑task to meet deadlines.
