Job summary

Turning Point of Central California, Inc. seeks a Mental Health Specialist/Case Manager IV to provide targeted services to clients, including crisis assessments, case management, and linkage to housing, medical, and educational resources.

You will work in settings such as client residences, schools, and tele-health, documenting in EHR, and collaborating with community partners. A Bachelor's in social work or counseling and Spanish bilingual ability are required.

JOB SUMMARYThe Mental Health Specialist/Case Manager IV reports to the Program Director and provides specialized services to clients in gaining access to needed medical, social, housing, economic, educational, and other direct services.ESSENTIAL JOB RESPONSIBILITIESWill provide mental health services to children, youth and care providers in settings such as client residence, school, community locations, tele-health and clinic sites.Will provide mental health services aligned with assessment and care plan developed by a mental health professional.Will provide mental health interventions that focus on behavioral improvements such as teaching social skills, coping skills, anger managements skills to help clients achieve treatment plan goals.Will provide case management services intended to reduce barriers to treatment and provision of needed services through linkage to other community partners.Will collaborate with community partners as part of mental health activities.Will complete progress notes documenting service provision in electronic health record (EHR) in timely manner and in accordance to state/county standards.Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse and Tarasoff warnings per California reporting laws.Will manage referrals via telephone, walk-in, fax or email.Ensure all new referrals are logged and entered into the Electronic Health Record (EHR) in a timely manner.Meet with parents/guardians to complete initial admission documentation.Will provide crisis evaluation assessments, including the use of de-escalation techniques, develop contingency plans, make recommendations for involuntary hold placement and interface with county psychiatric emergency team staff. Will enter new episodes in the E.H.R. as part of crisis documentation.Will provide therapeutic behavioral services (TBS) to consumers, as assigned by Team Leader.Will provide personal service agreement (PSA) services, as assigned by Team Leader.Will attend job-related trainings and also participate in a variety of in-service training, seminars, and practicum.Will attend weekly staff meetings and weekly supervision meetings.Will think and act quickly and efficiently in emergencies.Will drive on Agency business.Physical presence at the office is required.ESSENTIAL JOB REQUIREMENTSBachelor’s degree in social work or counseling in a related field providing direct client services in a mental health, social service, educational or similar environment.One year (1) experience working in a similar or related field, preferred.Communicate effectively in written and spoken English.Ability to communicate effectively in written and spoken Spanish and/or in Asian and Pacific Languages preferred.Work effectively with diverse ethnic, religious, socioeconomic and professional groups of people.Ability to operate within Electronic Health Records (EHR) system.Ability to operate tele-health platforms.Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws.
