The Full Time Social Worker role supports patients with social work and discharge planning in a busy psychiatric hospital.

You will facilitate therapeutic groups, provide counseling, and collaborate with a multidisciplinary team. A Master’s in Social Work (or related field) and 2+ years of experience are preferred;

Qualifications

• MS degree in social work, psychology or sociology.
• Active member of the Clinical Services/Social Work department.
• Experience facilitating therapeutic process and psychoeducational groups.
• Maintains a therapeutic relationship with patients.
• Two or more years in a Social Work environment preferred.
• Able to build and maintain professional helping relationships and communicate with diverse populations.

Responsibilities

• Provides social work and case management services for patients.
• Facilitating therapeutic process and psychoeducational groups.
• Completes biopsychosocial, discharge/aftercare planning and liaisons with community resources.
• Provides case management to assigned patients and provides group, family and individual counseling sessions as prescribed.
• Provides supervision to interns.
• Responsible for documentation of individual, family and group counseling sessions with final endorsement authorization.
• Plan, coordinate, manage and implement support packages to help clients deal with difficulties and overcome dependencies.
• Interview service users and assess their current condition, needs, strengths and weaknesses.
• Address each case as a unit and set tailored measurable goals.
• Monitor and evaluate clients’ progress and modify treatment plans accordingly.
• Offer information and counseling on the best course of action during sessions.
• Maintain accurate records and report on clients’ status.
• Act as a key-worker and cooperate with multidisciplinary teams
