Job summary

Alluma in Crookston, MN is seeking an Adult Services Rehabilitation Worker to deliver psychiatric rehabilitative services in clients' homes and the community. You will work under the direction of a Mental Health Professional to support recovery goals, coordinate with county providers, and help clients access housing and public services.

Responsibilities include developing treatment plans, facilitating daily living activities, ensuring client safety, and participating in joint treatment planning

Qualifications

• Must be at least 21 years old and have a high school diploma or equivalent.
• Experience with behavioral sciences or related fields preferred.
• Must have access to a personal vehicle and a valid driver’s license.
• No substance use problems for at least two years.
• Ability to work in clients' homes and community settings.

Responsibilities

• Assist clients in developing a treatment plan aligned with recovery goals.
• Assist clients with activities supporting daily living.
• Coordinate with county social services and other providers for joint treatment planning.
• Assess client safety in the community and plan referrals as needed.
• Help clients secure housing and access housing assistance.
• Assist clients in accessing public assistance and completing necessary paperwork.

Salary Range: $21.07 - $23.49 Hourly

As a Partner in Wellbeing, the Adult Services Rehabilitation Worker, under the direction of the Mental Health Professional, has direct contact with clients and is responsible for delivering specific psychiatric rehabilitative services to clients. Services to clients are primarily delivered in clients’ homes and/or the community and may be focused on the individual, the individual’s support team, and/or in groups.

Responsibilities

• Assist clients in developing a treatment plan consistent with their recovery goals.
• Assist clients, as needed, in successfully completing activities that support structured daily living.
• Coordinate with collaborative providers (e.g., county social services) to provide individualized client services and participate regularly in joint treatment planning with outside providers to work towards the recovery goals of each client.
• Assess for client safety in the community and develop a plan with the client, client’s team, and your supervisor to maintain safety and/or provide necessary referrals.
• Assist clients in anticipating their housing needs, locating safe, affordable housing, accessing and applying for housing assistance, and maintaining housing once obtained.
• Assist clients in accessing appropriate community services, applying for public assistance programs, and following through with necessary appointments and paperwork for these services.

Qualifications & Requirements

• Mental Health Rehabilitation workers must be 21 years of age, have a s high school diploma or equivalent, and meet one of the following:
• Be fluent in the non-English language or competent in the culture of the ethnic group to which at least 20 percent of the mental health rehabilitation worker’s clients belong or
• Have an associate of arts degree, or
• Have two years of full-time postsecondary education or a total of 15 semester hours or 23 quarter hours in behavioral sciences or related fields, or
• Be a registered nurse or
• Have, within the previous 10 years, three years of personal life experience with mental illness or
• Have, within the previous 10 years, three years of life experience as a primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability, or
• Have, within the previous 10 years, 2,000 hours of work experience providing healthand human services to individuals
• Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out position responsibilities
• Valid driver’s license and insurable under the agency’s automobile insurance policy
• Free from substance use problems for no less than two years
