Job summary

Alluma in Crookston, MN seeks a Mental Health Rehab Worker to provide rehabilitative services to adults with mental illness or emotional disturbance, aiding psychiatric stability and independent living skills.

You will conduct assessments, educate on mental health issues, and implement treatment plans under supervision. A valid driver’s license and reliable vehicle are required, with a 2-year substance-use-free period. Sign-on bonus offered and a background check required.

Qualifications

Responsibilities

• Provides Mental Health-services as outlined in the client’s treatment plan in the home, community, school, or office to help clients cope with and recover from the symptoms of their mental health condition.
• Practitioners may provide services to individuals, couples, families, or groups.
• Provide adequate documentation of services.
• Provide Targeted Case Management as a Case Management Associate to individuals with serious and persistent mental illness and/or severe emotional disturbance as determined.
• Complete initial and ongoing mental health outcomes measurements as well as implement treatment plans.
• Complete and update intake paperwork, screeners, functional assessments, releases, and forms/paperwork as needed.
• Assist clients, as needed, in successfully completing activities that support structured daily living.
• Participate in clinical supervision and team meetings as well as Interdisciplinary care coordination meetings.
• Schedules time effectively to ensure assigned billable hours targets are met.
• Participates and works collaboratively in all care coordination activities concerning clients on caseload as indicated by the care coordination managing the care-coordination function.
• May provide crisis assessments and interventions.
• Focuses on integrated health care, ensuring clients are referred to any necessary internal and/or external services to support their overall well-being.
• Ensures care coordination activities are client-centered.
• Participate and represent Alluma at community events.
• Performs additional duties as assigned or requested.

About the Role

As a Partner in Wellbeing, the Mental Health Rehab Worker, under the direction of the mental health professional or practitioner, provides rehabilitative services to adults with mental illness or children with emotional disturbance in the form of instruction, support, and assistance for the purpose of promoting psychiatric stability, social competencies, personal and emotional adjustments, and independent living skills. Duties include completing functional assessments and level of care assessments, education on mental health issues, completing crisis screeners and various screening tools, providing coping skills education and modelling, prompting, and observing clients to meet their activities of daily living. Mental Health Rehab Workers, under the oversight, direction, and evaluation of a mental health professional or practitioner, may provide rehabilitative mental health services to an adult client according to the client’s treatment plan.

Essential Responsibilities

QualificationsRequired Education, Certification, and Experience

• Must have a high school diploma or equivalent and meet one of the following:
• Be fluent in the non-English language or competent in the culture of the ethnic group to which at least 20 percent of the mental health rehab worker clients belong, OR
• Have an associate of arts degree OR
• Have two years of full-time postsecondary education or a total of 15 semester hours or 23 quarter hours in behavioral sciences or related fields OR
• Be a registered nurse, OR
• Have, within the previous 10 years, three years of personal life experience with mental illness OR
• Have within the previous 10 years, three years of life experience as a primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability, OR
• Have, within the previous 10 years, 000 hours of work experience providing health and human services to individuals.
• Must pass a criminal background check through MN DHS NetStudy.
• Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out the position’s responsibilities.
• Valid driver’s license and insurable under the agency’s automobile insurance policy.
• Must be 21, if the position provides you with an agency vehicle.
• Free from substance use problems for no less than 2 years.
