Job summary

Lake Superior Community Health Center, Inc. is seeking a behavioral health professional to provide integrated behavioral health services within a primary care setting.

The role includes assessments, diagnoses, counseling, and collaboration with the care team to support positive behavioral change. You will coordinate care with primary providers, participate in crisis intervention, and ensure thorough documentation in the EHR while adhering to HIPAA and OSHA regulations.

Qualifications

• Ability to work with an interdisciplinary team and provide services to diverse populations.
• Ability to evaluate, diagnose, and counsel clients independently with sensitivity and professionalism.
• Proficiency in brief, solution-focused behavioral health interventions and documentation in EHR.

Responsibilities

• Coordinate and co-manage individual patient care with rapid feedback to primary care providers.
• Provide on-site crisis triage and intervention as needed.
• Deliver brief behavioral health care and follow-up for positive behavioral change.
• Provide clinical consultation to medical providers on behavioral health and case management.
• Document assessments, diagnosis, treatment, referrals, and follow-up in the EHR according to guidelines.
• Comply with policies, standards of care, HIPAA and OSHA regulations.
• Participate in staff meetings, quality improvement, and collaborate with community organizations.
• Other duties as assigned.

Job description

Summary:

Provide behavioral health services and interventions to targeted clients and consultation to medical providers and related care team members utilizing an integrated behavioral health model.

Job Knowledge, Skills and Abilities:

• Ability to work with interdisciplinary team.
• Ability to provide service to persons of all economic levels and cultural backgrounds in sensitive, non-judgmental way.
• Ability to evaluate, diagnose, and counsel clients independently.
• Ability to provide brief, solution-focused behavioral health interventions.
• Excellent oral and written communication skills.
• Ability to respond to multiple demands and prioritize.

Essential Duties and Responsibilities:

• Coordinate, co-manage individual patient care, including consistent and expedited feedback to primary care provider
• Provide on-site crisis triage and intervention as needed
• Provide brief, solution-focused behavioral health care for LSCHC patients, including assessments and intervention geared toward positive behavioral change
• Provide clinical consultation to medical providers and related team members on behavioral health, case management, motivational interviewing and related issues
• Provide behavioral health assessment, diagnosis, and treatment, including needed referrals and follow-up care
• Maintain proper documentation in EHR according to clinical guidelines and operational procedures
• Comply with organizational policies, procedures, standards of care and related laws and regulations, including HIPAA and OSHA regulations
• Miscellaneous Duties
  • Actively participate in staff meetings and quality improvement initiatives
  • Collaborate with relevant community organizations to establish resource networks
  • Other duties as assigned

Licensure and Credentials:

• Working towards or fully licensed LICSW, LCSW, LMFT, LPC, or LPCC.
• Eligible for licensure in MN or WI
