Job summary

StoneSprings Hospital Center in Loudoun County, VA, is seeking a Mental Health Professional II to provide supervised mental health services, including assessments, psycho-education, and therapy across individual, family, and group sessions.

You will develop treatment plans, lead process groups, monitor safety, coordinate discharge planning, and perform post-discharge follow-up, collaborating with a multidisciplinary team.

Qualifications

• Masters degree in a relevant clinical field.
• Active clinical licensure preferred or required.
• Minimum 2 years of related experience.

Responsibilities

• Conducts one-on-one, group, and family psychoeducational and process groups.
• Develops treatment plans and attends planning meetings.
• Assesses environment and patient safety; reports issues.
• Identifies risk factors and interventions for high-risk patients.
• Completes assessments and documentation promptly and compliant.
• Collaborates with the multidisciplinary team for discharge planning.
• Conducts post-discharge follow-up as indicated.

Job Summary And Qualifications

The Mental Health Professional II is to deliver exceptional, patient-focused care by providing supervised mental health services to patients and their loved ones. Their responsibilities include conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy.

What you will do in this Role

• Conducts one-on-one, group, and family psychoeducational and process groups.
• Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
• Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Completes assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.
• Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
• Conducts post-discharge follow-up as indicated.

What qualifications you will need

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• Masters Degree
• 2 years experience
