Position Location

SC Department of Mental Health (DMH), Anderson, SC.

Job Summary

Under the supervision of the Mental Health Professional II, within the Office of Transitions Programs, the Transition Specialist identifies and executes activities to facilitate patient discharge from inpatient settings to community residences and behavioral health services. Responsibilities include discharge plan development, patient assessment, and linking patients to community providers.

Key Responsibilities
  1. Quality Management: Conduct clinical assessments, coordinate residential options, collect performance data, document services using Avatar and EMR, and prepare reports.
  2. Leadership: Coordinate with community providers, facilitate patient introductions, and participate in treatment and discharge planning meetings.
  3. Initiative: Maintain an inventory of community residences and develop resources to address gaps.
  4. Skill Enhancement: Complete required trainings, maintain licensure, and stay updated on policies and procedures.
Qualifications

A master's degree in guidance, counseling, social sciences, or related field, with 6 months to 1 year of clinical experience. Bilingual abilities in English and Spanish are a plus.

Physical Requirements

Must be able to perform duties that include bending, stooping, climbing, driving, lifting, and filing, across multiple settings.
