Tailored Care Management is a service which helps clients meet their health goals by coordinating their medical, social and behavioral health services. The Care Manager QP role is to improve positive client outcomes through linking clients to medication management, coordinating primary and behavioral health care needs, build trust between clients and their practitioners, connecting clients to community resources, enhance communication and the continuity of care.

Job Duties Include
- Completing the care management comprehensive assessment.
- Facilitation of case conferences.
- Ensuring that medication monitoring and reconciliation occur.
- Continuous monitoring of progress toward the goals identified in the care plan or ISP.
- Managing care transitions, including creating 90-day transition plans.
Basic Requirements
- Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
- Valid driver’s license, auto insurance coverage and reliable transportation.
- Pass criminal background check and other regulatory checks.
- Provide proof of your education.
Education And Experience Required
- A bachelor’s degree in Human Services, Social Work, Special Education, or other closely related Human Services area.
- Two (2) years of professional experience with the adult mental health population.
- Must have care coordination, care management, or case management experience.
Resume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served.
