GENERAL OVERVIEW:

The Mental Health Professional will provide clinical treatment services to targeted populations. This outpatient treatment program serves patients with mental health diagnoses who meet criteria for outpatient and/or partial hospitalization treatment.

ESSENTIAL RESPONSIBILITIES:

  1. Develops treatment plans for physicians that include a continuum of therapeutic, supportive, and other services tailored to patients' needs and diagnoses. (40%)
  2. Collaborates with the clinical team to determine the need for additional services and refers patients to appropriate levels of care, community self-help programs, or social services. (10%)
  3. Documents assessments, treatment sessions, contacts, case management, and other patient-related work in accordance with guidelines. (10%)
  4. Participates in quality assurance and performance improvement activities. (5%)
  5. Monitors and updates BH Treatment Plans, informing physicians as needed. (10%)
  6. Acts as a liaison internally and externally to ensure patient care requirements are met, participating in meetings as assigned. (5%)
  7. Conducts assessments of new patients, documenting findings and making clinical decisions regarding treatment, interventions, and referrals. Implements and adapts treatment models for service delivery. (20%)

QUALIFICATIONS:

Minimum:

  • Master’s degree in Psychology, Social Work, Counseling, Education (School Counseling), or related field
  • CPR certification from the American Heart Association
  • Act 34 Criminal Background Clearance
  • Act 33 Child Abuse Clearance
  • Act 73 FBI Fingerprinting Criminal Background Clearance
